Why iron becomes the primary nutritional bottleneck at six months
Healthy, full-term infants are born with an iron reserve accumulated primarily during the third trimester of pregnancy. Nature designed this reserve to sustain the infant through the first half-year of life.
By approximately six months of age, those congenital iron reserves are largely exhausted. At the same time, breast milk—while exceptionally rich in bioactive enzymes and antibodies—contains very little iron (roughly 0.35 mg per litre). While breast milk iron has high bioavailability (around 50% absorption), the absolute volume is no longer sufficient to meet the rapid blood volume expansion and brain myelination of a six-month-old infant, who requires approximately 7 to 11 mg of iron per day.
Without dietary complementary iron, infants become vulnerable to iron-deficiency anaemia between 6 and 12 months. This is why international health organisations (WHO, AAP, NHS) stress that complementary feeding must prioritise iron-dense foods from day one.
Haem vs non-haem iron: the absorption gap
Dietary iron exists in two distinct biochemical forms:
- Haem iron: Found exclusively in animal proteins (beef, lamb, liver, dark chicken/turkey meat, fish). Haem iron is absorbed efficiently by the human intestine (15% to 35% absorption rate) and is largely unaffected by other dietary compounds.
- Non-haem iron: Found in plant foods (lentils, beans, chickpeas, spinach, fortified grains, seeds). Non-haem iron has a lower absorption rate (2% to 10%) and is strongly influenced by absorption enhancers and inhibitors present in the same meal.
Both sources are valuable, but families offering plant-based meals must combine ingredients intentionally to achieve adequate absorption.
The problem with traditional refined rice cereal
For fifty years, parents were told that iron-fortified white baby rice cereal was the only proper first food. Modern paediatric feeding guidance has largely moved away from this recommendation for two reasons:
- Inorganic arsenic accumulation: Rice plants naturally absorb inorganic arsenic from soil and water at higher rates than other grains. Health agencies, including the US FDA and EFSA, recommend varying infant grains (incorporating rolled oats, quinoa, buckwheat, and barley) rather than relying exclusively on rice cereal.
- Nutritional monotony: Refined rice cereal is bland and sweet, teaching infants to expect homogenous textures rather than developing familiarity with savoury proteins and fibrous vegetables during the critical flavour window.
The best first iron sources for babies

Bioavailable haem sources:
- Slow-cooked beef or lamb: Braised until fall-apart tender. Shred finely for finger feeding or purée with bone broth.
- Dark chicken or turkey thigh: Dark poultry contains roughly double the iron of white chicken breast. Offer a soft, shredded drumstick portion or blend into mash.
- Canned sardines (in spring water, no salt): Mash whole sardines (bones are soft and supply calcium as well as iron and omega-3 DHA) on soft wholewheat toast.

Plant-based non-haem sources:
- Lentils (red or brown): Cook into a thick, soft dahl or stew with sweet potato and garlic.
- Chickpeas and black beans: Mash lightly with a fork or blend into homemade salt-free hummus.
- Ground pumpkin seeds and hemp hearts: Sprinkle a teaspoon into warm oatmeal or plain whole-milk yoghurt.
- Fortified rolled oats: Cook with water or breast milk and top with crushed raspberries.
How to use vitamin C to double plant iron absorption
Non-haem plant iron exists in the ferric state ((Fe^{3+})), which must be reduced to ferrous iron ((Fe^{2+})) before intestinal absorption. Vitamin C (ascorbic acid) is the most potent chemical enhancer of non-haem iron absorption.
Adding a source of vitamin C to a plant-based iron meal can increase non-haem iron absorption by up to 300%. Simple pairings in the kitchen:
- Stir steamed broccoli florets into lentil stew.
- Mash fresh strawberries or kiwi into fortified oat porridge.
- Squeeze fresh lemon juice over black bean purée.
- Serve soft roasted bell pepper strips alongside hummus toast.
Three common iron blockers to keep away from meals
Just as vitamin C boosts non-haem iron absorption, certain dietary compounds inhibit it:
- Cow's milk as a beverage: Calcium competitively binds to the DMT-1 transporter in the infant gut, blocking both haem and non-haem iron uptake. While plain yoghurt and pasteurised cheese can be part of meals in moderation, cow's milk should never replace breast milk or formula as a primary beverage before 12 months.
- Unprocessed bran and high-phytate grain skins: Phytic acid binds to iron molecules in the gut, forming insoluble complexes. Choose soaked, cooked whole oats and sprouted grains rather than adding raw wheat bran to baby food.
- Polyphenols: Teas and herbal infusions should never be given to infants, as their tannins bind dietary iron almost completely.
