Most baby food advice is flexible: purees or finger foods, mornings or afternoons, avocado or sweet potato. But there are three ingredients where paediatric guidance worldwide is completely unanimous and non-negotiable: no honey, no added salt, and no added sugar before age one.

These are not arbitrary lifestyle preferences. Each ban is based on specific physiological limitations of an infant's developing organs. Here is the medical science behind why these three stay out of the kitchen.

1. Honey and the danger of infant botulism

Honey is naturally antimicrobial for adults and older children, but for a baby under 12 months, it is a dangerous biological risk. Honey frequently contains microscopic spores of the bacterium Clostridium botulinum, picked up by bees from soil and pollen.

In adults and children over one year, a mature gut microbiome and acidic stomach secretions easily outcompete and destroy these dormant spores. In an infant under one year, the digestive tract lacks the competitive microbial diversity to suppress them. The spores germinate in the infant intestine, colonise the gut, and produce a potent neurotoxin that blocks acetylcholine release at the neuromuscular junction.

The result is infant botulism, a rare but life-threatening neurological condition characterized by:

2. Salt and the infant kidney limit

An adult's kidneys are powerful filtration engines capable of handling excess sodium by excreting concentrated urine. An infant's kidneys, however, are anatomically immature. The glomerular filtration rate (GFR) in a six-month-old is only about half that of an adult.

3. Sugar and taste receptor programming

Unlike salt and honey, a pinch of sugar won't cause immediate organ failure or bacterial toxicity. The ban on added sugar before age one is about neurodevelopment and metabolic programming.

Human infants are born with an evolutionary preference for sweetness, because in nature sweet meant calorie-dense and non-toxic (like maternal milk).
